This program fills the service gap that is not addressed by Ryan White Part B and seeks to increase access to care and promote optimal health outcomes for our clients, especially those who are uninsured or underinsured. Our Case Managers assist clients who need medication assistance through manufacturer copay cards, Mail Meds, or PAF (Patient Assistance Foundation), etc. Our Case Managers assist/refer clients in completing the eligibility process so that they can receive their medications at no cost, or significantly reduced cost.
We also refer some clients in need of assistance with other medical services through Project Hope, Bay Cares, Avicenna Clinic, etc.
